It's Jewel's observation of a gay man who is afraid to be open about his sexuality. The men at the bar are drawing you to believe they are not accepting of homosexuality and he is afraid of them finding out about him.
As a gay man, he would not covet a woman's breasts like a heterosexual man would and thus, it would be like eating something distasteful for him to look at them in that regard.

I remember the first time I read this poem. It was so simple, yet spoke volumes. I believe a lot of people relate to it. Once again, Jewel shows her undaunted understanding of the human condition.
